Actor Harrison Ford, who has been involved in a series of crashes and near-crashes while piloting aircraft, was involved in a “potentially serious incident” Monday that is under investigation by the Federal Aviation Administration, NBC News reports.

The latest incident took place while Ford was flying his private plane, a single-engine Husky.

“Ford, an experienced pilot who collects vintage planes, had been instructed to land on runway 20-L at John Wayne Airport in Orange County, California, but mistakenly aimed for a taxiway, instead. His plane passed over the top of an American Airlines 737 carrying 110 passengers and a six-person crew,” NBC News reports.

Moments after the incident, the passenger plane, AA flight 1546, departed safely for Dallas.

“Ford, 74, was heard on air traffic control recordings asking, ‘Was that airliner meant to be underneath me?,'” the story reports. “Air traffic controllers then informed Ford that he had landed on a taxiway rather than the runway — a violation of Federal Aviation Administration safety rules.”

The FAA indicated that Ford received the proper landing instructions from controllers.

“The incident has prompted an FAA investigation — which could result in a simple warning letter to a suspension of Ford’s pilot’s license,” NBC News reports.

In an incident that made headlines in March 2015, Ford was injured when he crash-landed a vintage single-engine World War II training plane into a golf course near the Santa Monica Airport.

In that incident, the plane reportedly stalled during takeoff from the Santa Monica Airport, forcing Ford to make an emergency landing.

The National Transportation Safety Board later determined that the 2015 crash was not Harrison’s fault, noting that a loose part led to the failure of the plane’s engine.
